Repeat and re-testing guidance

“Repeat testing.
Repeat testing refers to additional testing performed for an individual immediately following a first test that shows inconclusive or discordant test results during the same testing visit. The same assays are used and where possible the same specimen.
Re-testing.
This refers to additional testing performed for an individual after a defined period of time for explicit reasons, such as: A specific incident of possible HIV exposure with the past three months (window period); Or ongoing risk of HIV exposure. Re-testing is always performed on a new specimen.
Re-testing recommendations.
1. Based on indeterminate results: e.g. Discordant results: Re-test after two weeks.
2. Based on population setting: Generalized epidemic. Pregnant - re-test in third trimester or in labor. Symptomatic patients (STI and TB) - Re-test after 4 weeks and in every new episode of STI.
3. HIV negative persons with on-going risk behaviors: Re-test annually.
i) IDUs, MSMs, Sex workers; ii) Persons with HIV-positive partner; iii) Persons with partners of unknown HIV status.
4. HIV negative persons who have had a specific incident of known HIV exposure within the past three months: Re-test after 4 weeks.
5. HIV negative persons who have had a specific incident of possible HIV exposure with the past 72 hours: Re-test after 4 weeks and 12 weeks.
